Conduct in-home patient assessments and develop personalized care plans.
Administer medications, wound care, and treatments as prescribed.
Educate patients and families on managing health conditions and promoting independence.
Monitor patient progress and update care plans as needed.
Supervise LPNs and Home Health Aides to ensure the highest quality of care.
Collaborate with an interdisciplinary team to coordinate holistic patient care.
Active Registered Nurse (RN) license in the State of Arizona.
Minimum of one year of nursing experience ( home health, hospice, or critical care preferred ).
CPR certification and valid driver’s license with reliable transportation.
Excellent communication, organizational, and interpersonal skills.
