**Job Description: Director of Transplant Services**
The Director of Transplant Services helps meet health goals related to their specific program. They help to create a solid foundation and build the necessary support systems for the program to run well. Their work includes developing training for health professionals, educational programs for patients, and community outreach activities to make the program more effective. They also manage assigned staff.
**Job Responsibilities:**
1. Along with the Program Medical Director, ensures that the program runs well and efficiently. They work to meet goals that improve patient care. They also lead important projects and assess how well the program is doing, including any grants.
2. Works with the Program Medical Director and other leaders to involve stakeholders in using best practices and standard forms. This also involves building better relationships with outside partners to improve care quality and financial outcomes.
3. Creates and puts into action strategies that offer a great patient experience. They aim to make care more coordinated and consistent with both state and national guidelines.
4. Promotes teamwork and clear communication among all stakeholders, including leaders, providers, staff, and the community, by forming partnerships and creating effective ways to communicate.
5. Makes processes simpler and more reliable for both staff and patients. Works to improve access to care and reduce unnecessary emergency visits by using proven methods.
6. Works with committees to ensure the programs maintain necessary accreditations and recognitions from relevant agencies and organizations.
7. Co-facilitates meetings with the assigned program team, including the nursing director and medical director.
8. Works with leadership on quality improvement initiatives and other certifications needed for the program.
9. Collaborates with leadership to engage in programs by the Centers for Medicare & Medicaid Innovation and other payment models that improve patient access to care.
10. Helps find and build partnerships within the community to improve patient engagement and access to community resources as needed.
11. Represents the facility at national and local organizations, initiatives, and conferences related to the program to promote professional excellence, networking, partnerships, and patient initiatives.
**Skills and Abilities Needed:**
- Understanding of the professional guidelines and standards of the program.
- Experience with program oversight and data management systems.
- Skills in evaluating program processes and results while aligning with the organization’s mission.
- Ability to manage multiple clinical teams and ensure strategic plans are put into action.
- Strong communication skills for working with doctors, nurses, and staff.
- Leadership skills to motivate, recruit, and train staff.
- Commitment to providing a great customer experience through respect, listening, and clear communication.
- Capability to build community partnerships for better patient engagement.
- Knowledge in managing resources, including budget and personnel.
- Good writing skills for making grant proposals, course outlines, manuals, and reports.
- Proficiency in using computers for word processing and spreadsheets.
Experience:
Education: Must have a Master's degree in Nursing, Business Administration, Health Administration or related field. - Required.
Experience: Must have 8+ years of professional experience in Transplant Services and Peritoneal Dialysis operations. - Required.
